NC Sexual Assault Evidence Collection Kit (SAECK)

The TriTech kit is a little wider and a little thinner than the previous kit. The tracking kit serial number leads with "T".

The SAECK includes the following paperwork:

• Patient Information Sheet (English & Spanish)

• Authorization for Treatment, Examination, Collection and Release of Evidence and Information- Sexual Assault Investigation (triplicate form in English & Spanish)

• N.C. Sexual Assault Data Form (triplicate form)

• Evidence Checklist & Disposition of Evidence (triplicate form)

• Anatomical Drawings

• Medical History Form- Page 1

• Medical History Form- Page 2

• Kit re-order form

​Make sure that all clothing or sanitary items are dry before sealing.

• Outer Clothing

• Underpants

• Sanitary Pad/Tampon

Known Standard Envelopes:

• Known Cheek Scraping

• Pubic Hair Standard

Unknown Envelopes:

• Oral Swabs

• Pubic Hair Combings

• External Genitalia Swabs

Blood & Urine should be collected any time there is concern for a possible drug-facilitated assault.

• 2 plastic bags with chain of custody labels for blood and urine samples​

• Bags do NOT go back into the SAECK, they should be refrigerated until processed

Additional items in the kit include:

• Swabs (make sure they are NOT expired)

• Plastic swab dryers

• Evidence tape for each envelope 

• Evidence tape for the outside of the box

• Biohazard label for the outside of the box
